We couldn’t have lived without the video monitor (and even now we can’t–I have no intention of EVER taking it out of their rooms). We also relied on the co-sleeper bed beside ours so that I didn’t have to get up to go get either one to nurse at night.
Hand me downs are a great way to acquire the necessary stuff but be careful that there are not any recalls or safety upgrades. Never get a used car seat! I would also encourage you to think about BPA-free bottles and cups…just a suggestion.
I second the need for Lansinoh and tell your wife she needs more pads (both breast and maxi) than she could ever, ever imagine on hand. (TMI…but you will get used to it all, i promise:))

I know it sounds cliche, but newborns need little more than food and warmth.
But . . . I know that’s not *really* the answer you’re looking for. I have three kids, so we had the “must haves” down to a science by the time #3 came along.
Lansinoh nipple ointment (if your wife will nurse). Because no matter what anyone says, it does HURT in the beginning.
a breast pump and bottles – because engorgement happens and babies don’t always feed when you need them to (again, assuming that your wife will nurse)
a bouncy seat, sling and/or swing – all of mine preferred something different, and it was usually the thing I didn’t have on hand at the time
